With Eyes Turned Skyward
On February 28, 2012, a US Coast Guard MH-65D Dolphin helicopter, CG-6535, crashed in Mobile Bay, Alabama, during a nighttime search and rescue training flight, resulting in the loss of all four crew members. The helicopter, assigned to the Coast Guard's Aviation Training Center in Mobile, was conducting practice hoists with a training vessel when the weather deteriorated. The crew, consisting of a pilot-in-command (PIC), a pilot under instruction, a flight mechanic, and a rescue swimmer, was using night vision goggles. The investigation found that the helicopter entered into an Inadvertent Instrument Meteorological Conditions (IMC), where the crew lost visual reference to the horizon. The crash tragically claimed the lives of Lt. Cmdr. Dale Taylor, Lt. j.g. Thomas Cameron, Chief Petty Officer Fernando Jorge, and Petty Officer 3rd Class Andrew Knight.
